Important note regarding registration using PayPal
Please make sure you add the following in the "Add special instructions to seller" link/field when checking out: RUSA # (please put 'pending', or 'none' if you do not have one), your home and emergency telephone numbers and if your PayPal shipping address is not the correct mailing address please include your address. If you are under 18 years old, please use the PDF format and request a waiver form for your parent or guardian to sign.
